REQUEST FOR QUOTATIONS
Project Identification:
3119 Rocky River Road, Omak, WA 98841
Walk-in Shower with grab bars, and access ramp.
1. Remove existing tub/shower.
2. Furnish and install AQUATIC REMODELINE 30 in x 60 in x 76 in Gelacoat shower stall (or approved equal) and SPEAKMAN Versatile ADA plus handheld shower system (or approved equal).
3. Install an access ramp and hand rail at side entrance.
The Colville Indian Housing Authority is requesting Quotations to furnish labor and materials for the above project.
All work and materials will be as shown in the specifications, plans, and provisions to the contract.
A form of non-collusive affidavit is included with the request for quotations and must be filled out completely and returned with quote.
A pre-quote walk through will be held at 3119 Rocky River Road, Omak, WA 98841 on August 1st, 2018 at 10 a.m.
CIHA will receive quotations no later than 3:00 PM (local time), August 9th, 2018. Quotations must be submitted on the Form of Quotation, along with all attached forms, filled out completely, as required in the Contract Documents and Specifications. All written quotations must be delivered, faxed or emailed to:

CIHA reserves the right to reject any and all quotations or to cancel or modify this RFQ at its sole discretion if it determines it is in the best interest of CIHA to do so.
Award of contract will be based on small purchase methods of procurement as spelled out in the Federal Register Rules and Regulations Vol. 60 No. 68 sec. 950.165.
Award based on price. CIHA shall make award to the qualified Indian-owned economic enterprise or organization with the lowest responsive quotation if it is reasonable and no more than 10% higher than the lowest responsive quotation received. If no responsive quotation from a qualified Indian-owned economic enterprise or organization is within 10% of the lowest responsive quotation from any qualified source, then award shall be made to the source with the lowest quotation.
Schedule of Completion. Schedule of completion for this project will be no more than 10 consecutive calendar days from start to finish.
All TERO and TOSH A regulations shall be adhered to in the performance of this contract work. Indian preference will be given in the award of contracts “to the greatest extent feasible” as determined in sec 7(b) of the Indian Self Determination and Education Act (25 U.S.C. 450e (b). Davis Bacon (WA109) wages will be in effect on this work. (See attached)
NOTE: Before a contract can be executed the Contractor and any sub-contractors must provide proof of liability insurance and auto insurance. They must provide Washington State Industrial Insurance or equivalent if the Contractor or Sub Contractor plans to hire employees.

1. Demo and remove existing concrete sidewalk.
2. Install 16’ ramp with a 5’x5’ concrete landing.
3. Paint stabilize four (4) interior doors and jambs.
The Colville Indian Housing Authority is requesting Quotations to furnish labor and materials for the above project.
All work and materials will be as shown in the specifications and drawings that are included with the SCOPE OF WORK AND PROVISIONS TO THE CONTRACT.
